Meet all of the following requirements to be eligible to join us for free:
A Medi-cal holder
The California Medical Assistance Program (Medi-Cal or MediCal) is California's Medicaid program serving low-income individuals, including families, seniors, persons with disabilities, children in foster care, pregnant women, and childless adults with incomes below the federal poverty level.
Must be a Medi-cal holder
CalOptima Health
Currently enrolled in CalOptima Care Plan
CalOptima Health
Functional impairments, either physical or mental
Has 2 or more diagnoses such as Hypertension, Diabetes, Insomnia, Alzheimer, Hyperlipidemia, Begigh prostatic hyperplasia, Depression, Aneity, Gastroesophageal Reflux Diseases, etc.

Self-Pay Option
If your are not eligible to receive our services for free, self-pay option is also accepted. Please call us at 714-215-4388 to discuss the individual self-pay option to join us.
