Available Services

Social Service

Social workers will provide supervision, assessment, and monitoring of the participant's psychosocial status to maintain and improve the participant's emotional well-being; conduct problem-centered support groups to address psychosocial issues; case management and care coordination services are also provided to help participants receive needed resources and maximize independence

PSYCHOLOGICAL/PSYCHIATRIC THERAPY

A licensed clinical social worker (LCSW) will provide education for emotional and mental health, counseling for personal, family, and social issues

Professional Nursing Service

provide onsite professional nursing services, which include general health monitoring and assessment, medication management, communication with participants' health providers; personal care services; skilled nursing services; assist participants in obtaining medical services; health education, and counseling when needed.

Meals

We provide professional dietitians with carefully prepared meals, complimentary breakfasts and lunches for participants, and snack breaks in between.

Nutrition Counseling

Nutritional meals are provided based on the physician’s order and the Registered Dietitian’s nutritional assessments and counseling. The center will also be tailored to the participant’s needs and preferences.

Physical & Occupational Theapy

Licensed Physical and Occupational Therapists will develop an individual care plan, and provides therapy, and personalized exercises and treatments to maintain mobility and strengthen endurance. education that promotes ongoing support in activities of daily living.

Entertainment Activities

Participants can enjoy various different activities of their own preferences on different days of the week for social engagement, cognitive stimulation, exercise, and entertainment. We also provide customized group or individual activities for those participants who are frail or have a cognitive functioning impairment. 

Field Trip

We offer trips with elders such as Grocery Shopping and outings to host special holiday events.

Transportation

We provide round-trip transportation for our members to commute to our center. Our service area is primarily in Orange County.

The completed AD-3027 form or letter must be submitted to USDA by:
